Understanding Benefit Routes
For insulin pumps and CGMs, the "Benefit Route" is the most important piece of information to find. It determines which department processes the claim, which suppliers you can use, and how much you will pay.
DME (Durable Medical Equipment)
Items are processed under the medical benefit, often requiring a specialized medical equipment supplier rather than a local pharmacy.
- Often has a deductible and 20% coinsurance.
- Requires a Detailed Written Order (DWO).
Pharmacy Benefit
Items are processed like medications at a retail or mail-order pharmacy. This is often faster and has more predictable copays.
- Usually processed via a tiered copay system.
- Fewer clinical documentation requirements in some cases.
Why it matters
If you try to fill a DME-only pump at a retail pharmacy, the claim will be rejected as "Non-Formulary" or "Plan Exclusion," even though the item is actually covered. Knowing the route prevents these false rejections.
